Create a ClickUp task to review every new SurveyCTO form submission
Each new SurveyCTO form submission creates a ClickUp task, so every response collected in the field lands in your team’s task list as something to review.

What this workflow does
Field teams send SurveyCTO submissions in throughout the day, and it is easy for some to sit unchecked. If your reviewers already plan their work in ClickUp, the review queue belongs there too. This workflow opens a ClickUp task for every new submission.
The SurveyCTO trigger fires for each new form submission and hands the submitted data to FlowMattic. The next step runs the Create Task action in ClickUp. You map submission values into the task, such as a respondent ID and the key answers, so a reviewer can tell what to check without leaving ClickUp.
Every submission gets its own task. The template does not check the answers first, so test entries and duplicates create tasks too unless you add a Filter. Run one test submission after import to see which values SurveyCTO passes and which task details the ClickUp step accepts. Task History shows each run and the response from ClickUp.
Quality checks on household surveys
Enumerators upload interviews from the field. Each one becomes a ClickUp task so a supervisor can check it before the data is used in analysis.
Site inspection follow-up
Inspectors file a SurveyCTO form for every site visit. The ClickUp task gives the office team one place to track what needs a response.
Monitoring and evaluation reviews
Program staff submit monitoring forms, and each submission shows up in the M&E team's ClickUp list to be reviewed and closed out.
How this workflow runs
What happens at each step once the workflow is on, and what you need before you import it.
SurveyCTO New Form SubmissionFires when a new submission arrives for your SurveyCTO form. The submitted values are passed on so you can use them in the ClickUp task.
ClickUp Create TaskCreates a new task in ClickUp for the submission. Connect your ClickUp account and map SurveyCTO values into the task. Check with a test run which task details the step lets you set.
What you need
- FlowMatticCore
- SurveyCTO AccountAccount
- ClickUp AccountAccount
After importing, map your own data. To keep the template universal, not every step is pre-filled. Open each step in the builder and connect your accounts and fields (channels, lists, sheets, recipients) before you switch the workflow on.
Make it yours
Changes worth making after you import it.
- Add a Filter after the trigger so tasks are created only for submissions that need a person, such as forms where a flagged question has a particular answer.
- Include a submission identifier from SurveyCTO in the task, so reviewers can find the matching record in SurveyCTO quickly.
- Use a Router in place of the single ClickUp step if different forms or regions should go to different reviewers' lists, with a condition per route that cannot overlap.
Questions about this workflow
Does every SurveyCTO submission create its own ClickUp task?
Yes. The trigger fires once per new submission and the Create Task step runs each time. Add a Filter after the trigger if you only want tasks for some submissions.
Do I need accounts for both SurveyCTO and ClickUp?
Yes. You connect a SurveyCTO account for the trigger and a ClickUp account for the task step when you set up the imported workflow.
Will SurveyCTO submissions from before the import become ClickUp tasks?
No. Only submissions that arrive after the workflow is turned on start a run. Older ones stay in SurveyCTO.
Does closing the ClickUp task change anything in SurveyCTO?
No. The workflow only sends data one way, from SurveyCTO to ClickUp. Marking a task done has no effect on the submission.
