What this workflow does
Form.io collects structured data well, but somebody still has to notice a new entry and act on it. When responses sit unseen in a dashboard, follow-up slows down. This workflow puts each one in front of your team in Slack.
The Form.io New Form Submission trigger fires when your form receives a new submission. FlowMattic passes the submitted data to Slack, where the Send Channel Message action posts to the channel you choose. You write the message and place form answers inside it, for example the person's name, their request and how to reach them.
Every submission posts once. For a private channel, Slack's separate Send Private Channel Message action is the one to use. To alert one person instead of a channel, swap in Send Direct Message.
Field inspection reports
Inspectors submit a Form.io report from site. The operations channel in Slack gets the key findings so a coordinator can schedule repairs.
Internal request forms
Staff request equipment or access through a Form.io form. The IT channel sees each request with the requester's name and what they need.
Application intake for a program
Applicants fill in a Form.io application. Reviewers in a Slack channel get a short summary and know to open the full entry.

Make it yours
Changes worth making after you import it.
- Add a Filter after the trigger that continues only when a priority or category answer matches, so the channel only hears about the submissions that need quick action.
- Use a Router to send different kinds of submissions to different channels. Move the existing Slack step into the first route and give each other route its own copy set to its channel.
- Keep the Slack message short and include the submitter's contact detail, so whoever picks it up can reply without opening Form.io.
Questions about this workflow
Can the Form.io alert go to a private Slack channel?
Swap the Slack step for Send Private Channel Message if the alert belongs in a private channel. The template's Send Channel Message step is meant for regular channels.
Does every Form.io form on my account post to Slack?
Check the trigger settings after import to see how the form is chosen, then send a test entry from the form you want to watch.
Will file uploads from Form.io appear in Slack?
The message holds the text you map into it. Check a test submission to see what Form.io sends for an uploaded file before you map it.
Are earlier Form.io submissions posted to Slack?
No. Slack only hears about Form.io entries that arrive once the workflow is switched on.
