AI agent that turns feedback into action across the org

Each Fluent Forms feedback entry goes to an AI agent that summarises it and picks what to do: it can thank the customer by email, file a ClickUp task for bugs or requests, and share praise with marketing in Slack.

The workflow as it appears in the builder after import. Each step opens for editing.

What this workflow does

Feedback forms collect a mix of complaints, ideas and compliments, and they often sit unread because no single team owns them. Bugs never reach the people who fix them, praise never reaches the people who could quote it, and the customer never hears back. This workflow lets an AI agent read each entry and send it where it belongs.

When someone submits your Fluent Forms feedback form, the AI Agent receives the rating and the feedback text. Running on OpenAI GPT-4o, it is told to summarise the feedback, thank the customer, and take the right action: create a ClickUp task for bugs or requests, or post praise to marketing. It has three tools. Email sends a thank-you to the address on the form, with subject and body the agent writes. ClickUp Create Task files an item with a name and description from the agent. Slack Send Channel Message shares a message in a channel you choose.

The agent decides which tools fit each entry, so one piece of feedback might lead to an email and a task, another to an email and a Slack post. Review Task History for the first submissions to check its choices, and refine the system prompt where they differ from what your team expects.

Post-purchase feedback form

Customers rate their order and leave a comment. Complaints about a product defect become ClickUp tasks, and glowing reviews are posted in Slack for marketing to reuse.

App feature requests

Users suggest features through the form. The agent writes each as a ClickUp task with a clear summary, and the user gets a thank-you email.

Event survey

Attendees send ratings and comments after an event. Praise reaches the marketing channel for social posts, while problems become tasks for the next event.

How this workflow runs

What happens at each step once the workflow is on, and what you need before you import it.

  1. Step 1Trigger
    Fluent Forms New Form Submission

Starts the workflow on each new Fluent Forms feedback entry, passing the rating, the feedback text and the customer's email to the agent and its tools.

  2. Step 2Action
    AI Agent Execute Agent

Gives the rating and feedback to an AI agent on OpenAI GPT-4o, told to summarise, thank the customer, and file bugs or requests in ClickUp or post praise to marketing.

  3. Step 3Tool
    Email Send Email

Agent tool: emails the customer at the address from the form. The agent writes the subject and body, which is how the thank-you is sent.

  4. Step 4Tool
    ClickUp Create Task

Agent tool: creates a ClickUp task for a bug or feature request. The agent writes the task name and the description from the feedback.

  5. Step 5Tool
    Slack Send Channel Message

Agent tool: posts a message in a Slack channel, written by the agent, to share praise or themes with your marketing team.

What you need

  • FlowMatticCore
  • Fluent Forms PluginPlugin
  • AI ProviderAccount
  • ClickUp AccountAccount
  • Slack AccountAccount

After importing, map your own data. To keep the template universal, not every step is pre-filled. Open each step in the builder and connect your accounts and fields (channels, lists, sheets, recipients) before you switch the workflow on.

Make it yours

Changes worth making after you import it.

  • Tell the agent in its system prompt how to use the rating, for example that a rating of 2 or lower should always become a ClickUp task, so its choices match your process.
  • Point the Slack tool at a dedicated channel such as your marketing or testimonials channel, so praise does not get lost in general chat.
  • Add a Filter after the trigger that stops runs when the feedback field is empty, so the agent is not asked to act on a rating with no comment.

Questions about this workflow

Does every Fluent Forms feedback entry get a thank-you email?

The agent is told to thank the customer, and it chooses when to call the Email tool. Check Task History early on, and make the instruction more explicit if any entry is missed.

What fields does my feedback form need?

The template maps rating, feedback and email. Name your fields the same, or remap the agent's input and the Email tool after a test entry.

Can feedback create a ClickUp task and a Slack post at the same time?

Yes, if the agent judges both fit. Its instructions describe tasks for bugs or requests and Slack for praise, so mixed feedback may use both tools.

Is the AI summary saved anywhere?

Only in the agent's output in Task History and in whatever it writes into the email, task or Slack message. Add a Google Sheets step after the agent to keep a log.
