Turn new JotForm submissions into items on monday.com boards
Each new Jotform submission becomes an item on your monday.com board through FlowMattic, so requests, orders or applications collected by a form are ready for your team to assign and track.

What this workflow does
Jotform is good at collecting information, and monday.com is where many teams decide who does what with it. Without a link between them, someone exports responses or retypes them onto the board. This workflow makes the board item as soon as a form is submitted.
The Jotform trigger New Form Submission picks up each new response to your form. FlowMattic passes the answers to Monday.com, where the Create Item action adds a new item to the board you choose. You map the form answers into the item name and the columns your board uses.
Every submission gets its own item, even when the same person fills in the form twice. The workflow does not change anything in Jotform, so your form's own emails and stored responses work as before. Task History keeps a record of each submission that went through.
Creative request intake
Staff request design work through a Jotform brief. Each brief appears on the design team's monday.com board, where it is assigned and moved through review.
Event volunteer sign-ups
Volunteers register on a Jotform page, and each one becomes an item on a monday.com board grouped by shift so the coordinator can plan cover.
IT and facilities tickets
Employees report a broken laptop or a leaking tap through a Jotform, and the request lands on the operations board as a new item.
How this workflow runs
What happens at each step once the workflow is on, and what you need before you import it.
Jotform New Form SubmissionFires when a new response is submitted to your Jotform form. All answers from that submission are passed on for mapping into monday.com.
Monday.com Create ItemAdds a new item to a monday.com board for the submission. Connect your account, pick the board, and map the form answers into the item name and columns.
What you need
- FlowMatticCore
- Jotform AccountAccount
- Monday.com AccountAccount
After importing, map your own data. To keep the template universal, not every step is pre-filled. Open each step in the builder and connect your accounts and fields (channels, lists, sheets, recipients) before you switch the workflow on.
Make it yours
Changes worth making after you import it.
- Submit a test response after import so FlowMattic captures every question on the form. Map from that sample to get the right answer into each column.
- Use a Router if one form feeds several teams. Give each route a condition on the request type and its own Create Item step pointing at that team's board.
- Put the submitter's name and a short subject in the item name, so the board stays readable at a glance, and keep the full answers in a text column.
Questions about this workflow
Does each Jotform response create a new monday.com item?
Yes. One submission means one new item. The workflow does not check the board for an existing item from the same person.
Can uploaded files from Jotform reach the monday.com item?
Check a test run with an upload. If the captured submission data includes a link to the file, you can map that link into a text or link column on the item.
Does this work with forms I already have in Jotform?
Yes. Connect your existing Jotform form by following the trigger's setup in FlowMattic after import. Only submissions made after the workflow is on are sent to monday.com.
Is anything written back to Jotform?
No. The workflow reads each submission and creates an item in monday.com. Your Jotform responses and notifications stay as they were.
