What this workflow does
Cognito Forms is a good place to take requests, but the work itself gets planned in ClickUp. Copying each entry into a task by hand slows things down and invites mistakes. This workflow creates the task for you every time a form is submitted.
The Cognito Forms trigger, New Entry, fires when a new entry is received on form submission. The answers then travel to ClickUp, and its Create Task action opens one fresh task per entry. You map the answers into the task name and description, for example the request type as the name and the full details below it.
Every entry creates one task, since there is no Filter. ClickUp doesn't check whether a similar task already exists, so a person who submits twice produces two tasks. Task History in FlowMattic shows each run and what was sent to ClickUp.

Will editing a Cognito Forms entry update the ClickUp task?
No. The trigger is New Entry, and the workflow only creates tasks. Changes to an entry after submission aren't sent to ClickUp.
