When a new response is submitted in Reform, create a task in ClickUp
Every new Reform response becomes a task in ClickUp, with the answers you choose mapped into it, so form requests sit in the same place as the rest of your team’s work.

What this workflow does
Form responses tend to wait in an inbox or a results page until someone turns them into work. If your team plans in ClickUp, that copy-and-paste step is where requests get lost. This workflow creates the task for you when the response arrives.
The Reform trigger starts a run each time a new response is submitted to your form. Every answer in it becomes data FlowMattic can pass on. The next step uses Create Task in ClickUp.
The template has no mappings yet. After import, connect both accounts, submit a test response, and decide where the task should go and what it should say. A common setup puts the respondent's name or request in the task name and the remaining answers in the description, but you can map any answer you like.
Each run shows in Task History, along with any error ClickUp sends back.
Design requests from clients
A studio's Reform brief form collects project details. Each brief appears as a ClickUp task the design lead can assign and schedule.
Internal IT or ops requests
Staff fill in a Reform request form for equipment or access. The operations team works through the requests as ClickUp tasks.
Content submissions
Guest authors pitch articles through Reform. Every pitch becomes a task for the editor to review in ClickUp.
How this workflow runs
What happens at each step once the workflow is on, and what you need before you import it.
Reform New ResponseFires when a new response is submitted to your Reform form. Connect Reform on import; each answer is then available to map into the ClickUp task.
ClickUp Create TaskCreates a task in ClickUp from the response. Connect ClickUp, choose where the task belongs, and map the answers into the task name and details after a test response.
What you need
- FlowMatticCore
- Reform AccountAccount
- ClickUp AccountAccount
After importing, map your own data. To keep the template universal, not every step is pre-filled. Open each step in the builder and connect your accounts and fields (channels, lists, sheets, recipients) before you switch the workflow on.
Make it yours
Changes worth making after you import it.
- Put the most useful answer, such as the request title or the client's name, in the task name so the task list is readable at a glance.
- Add a Filter after the trigger if only some responses need a task, for example when a request type answer equals Project.
- Add a Slack or email step after ClickUp if someone should hear about each new task as well.
