Create a ClickUp follow-up task and CRM contact for every new lead
Every Fluent Forms submission becomes a ClickUp follow-up task holding the lead’s email and message, and the same person is added to FluentCRM or updated if they are already a contact.

What this workflow does
A lead form is only useful if someone gets back to the person who filled it in. When submissions sit in the form plugin, follow-ups slip and the contact never reaches your email list. This workflow turns each entry into a task someone can own and a CRM contact you can email later.
The flow begins the moment a lead sends your Fluent Forms form. FlowMattic then creates a ClickUp task named "Follow up:" plus the lead's name, with the submitted email and message in the task description. After that, the FluentCRM step creates a contact from the email address and puts the submitted name in the first name, or updates the contact when that email is already in your CRM.
The template maps fields called name, email and message, and leaves the last name blank. If your form splits first and last names, change both mappings after import. FluentCRM works inside WordPress, so the contact stays on your own site, while the task is sent to your ClickUp workspace.
Consultation requests for a service business
Each booking enquiry gives a team member a ClickUp task to call back, and the person is added to FluentCRM for your newsletter or offers.
Agency new-business form
Prospects describe their project in the message field. The ClickUp task shows the brief to whoever replies, and FluentCRM keeps the lead for later campaigns.
Partner or reseller applications
Applicants are tracked in ClickUp until someone reviews them, while FluentCRM holds the contact so you can send partner updates.
How this workflow runs
What happens at each step once the workflow is on, and what you need before you import it.
Fluent Forms New Form SubmissionStarts the run when someone submits your Fluent Forms lead form. The name, email and message answers are handed to both action steps.
ClickUp Create TaskCreates a ClickUp task titled "Follow up:" with the lead's name. The description lists the submitted email and message. Connect ClickUp and check a test task to see where it lands.
FluentCRM Create or Update ContactCreates or updates a FluentCRM contact using the submitted email. The name field fills the first name and the last name is left empty, so remap both if your form collects them separately.
What you need
- FlowMatticCore
- Fluent Forms PluginPlugin
- ClickUp AccountAccount
- FluentCRM PluginPlugin
After importing, map your own data. To keep the template universal, not every step is pre-filled. Open each step in the builder and connect your accounts and fields (channels, lists, sheets, recipients) before you switch the workflow on.
Make it yours
Changes worth making after you import it.
- Add a Delay between ClickUp and FluentCRM if you want contacts added a little later, for example after your team has had a chance to review the lead.
- Make the email field required in Fluent Forms. FluentCRM needs an address to create the contact, and the ClickUp task is more useful with one.
- Add a Filter after the trigger that skips entries from your own email domain, so test submissions don't create tasks or contacts.
Questions about this workflow
What happens if the lead is already a FluentCRM contact?
The step uses Create or Update Contact, so the existing contact is updated rather than duplicated. A new ClickUp task is still created for every submission.
Does the ClickUp task get assigned to someone?
The template sets only the task name and description. Run a test after import to see which ClickUp options the step offers, and add an assignee there if it is available.
Why does the lead's whole name land in the FluentCRM first name field?
The template sends the whole name field to the first name and leaves last name blank. If your Fluent Forms form has separate first and last name fields, map each one in the FluentCRM step.
Do ClickUp and FluentCRM both need to be set up before import?
Yes. FluentCRM must be installed on the same WordPress site as FlowMattic, and you connect your ClickUp account in the task step.
